Beginning Dec. 22, Gulfstream Goodwill is accepting referrals for individuals who cannot safely isolate at home. The following is the process for accessing the COVID Shelter. The COVID Non-Congregate Shelter is located in Pahokee.
Individuals who do not have COVID and/or have potentially been exposed and are seeking placement in a Non-Congregate Shelter to safely self-isolate must meet the following criteria:
• Eligibility – individuals who need to be isolated and are unable to do so in their homes
• Self-attestation form to be completed by the individual
• Willingness to participate in wellness checks conducted by staff
• To continue to work, the person needs to isolate to minimize risk of exposure and cannot safely isolate at home because a household member has tested positive
• If an individual is of high risk of COVID-19 and cannot risk exposure from a positive family member and cannot safely isolate at home
• Individuals should bring hygiene materials, towels, clothing and other sundries
• Client must provide an emergency contact number for emergency purposes only.
If the person meets the above criteria, the person or referring agency can call 561-996-1690. At that time, arrangements will be made for person to arrive and meet staff at the Pahokee site. EVERYONE MUST
WEAR MASKS.
Individuals who have tested positive for COVID must meet the following criteria to be considered for placement in Non-Congregate Shelter:
• If individual absolutely cannot safely isolate at home AND
• Has tested positive for COVID-19 OR
• Is symptomatic and awaiting test result for COVID-19 OR
• Has potentially been exposed to COVID-19 and /or
• Client must be able to isolate independently as assessed by Human Services Branch case manager
• Client must have a recent positive test result (within 5 days)
• If client’s Discharge Order calls for medication, client must have a 14 day supply with them
• Client must provide an emergency contact number for emergency purposes only.
THEN follow the below process​:
1. To start process, call the EPI team at 561-671-4184
2. The EPI team will contact the ESF 8 Desk at EOC
3. The ESF 8 Desk will contact Human Services Branch
4. The Human Services Branch will contact the case manager to complete an assessment on potential client
5. Case manager will assess client for non-congregate site in Pahokee
6. Case manager will inform HS Branch and Goodwill of outcome of assessment and will contact GGI to make arrangements for person to enter shelter.
7. The individual must complete a self-attestation form upon entry to shelter. This can be done at admission.
Admission: Once eligibility is established, the client will meet a staff person at the shelter location. The client will be provided the key to a unit for self-quarantining.
